Statement of Fitness for Work - ‘Fit note’

The 'fit note' replaced the doctor's sick note on 6 April 2010. With your employer's support, the note may help you return to work sooner by providing more information about the effects of your illness or injury.

Your employer cannot ask you to provide medical evidence that you are sick for the first seven days of illness. During this period they may ask you to fill in a self-certificate of their own design or form SC2.

You only need a Doctor’s note if you are unable to work and are ill for longer than seven calender days. We ask that you make an appointment with the Doctor should your require a Medical Certificate, now called a 'Statement of Fitness for Work’.
Private Certificate

GPs are not obliged to issue NHS medical certificates for periods of sickness of less than seven days' duration. However, if you or your employer require a short-term certificate, the Doctor is entitled to charge a fee.
Please note that a charge of £15.00 will apply for this service.
